Working with dynamic professional staff in the Transitional Care Package Team, you will assist with management, clinical, and education activities within the Riverland Community Health Service. Maintaining up to date knowledge of Transitional Care Packages (TCP) and constant changes to the program, you will attend to regular meetings for Country Health SA TCP, undertake report writing for CHSA/CRU, and participate in goal setting for clients. You will also assist the Team Leader to maintain a learning culture by acting as a resource person, encouraging reflection and professional development, and assisting others to maintain portfolios/records of learning. Located near the fantastic Murray River and Lake Bonney in the Riverland Region, you will have the opportunity to enjoy real work/life balance in a welcoming rural community. You will be registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia, and hold or be eligible to hold a current practicing certificate. With demonstrated competence in delivering Transitional Care Package Programs or similar, you will have sound knowledge of goal setting and funding for TCP. You will have experience in performing clinical assessments, dealing appropriately and relevantly with clients and families where there are multiple complexities, diverse cultural backgrounds and expectations.
