  • Country Health SA Local Health Network, Whyalla
  • Indicative Total Remuneration: $86,052 - $99,495 – Temp F/T (up to 1/1/2018) – AHP2

You will contribute your expertise to the development and delivery of a comprehensive, integrated continuing rehabilitation service for the Whyalla community. Planning, implementing and evaluating services to Transition Care Package (TCP) clients, you will work as a member of a multidisciplinary team, including health professionals and service providers from other sectors, and utilise a combination of preventative, early intervention, treatment/therapy and evaluation approaches. Working under reduced clinical direction, you will contribute to the clinical supervision of less experienced allied health professionals, allied health assistants and students. You may also monitor the effectiveness of TCP services through episodic review, timely reassessments and evaluation against Key Performance Indicators. Make a real difference and feel like an important part of the community. Being part of Country Health SA means you will be supported both personally and professionally, can take advantage of education and training opportunities, will have access to flexible working hours and be part of an inter-professional approach to holistic health care.

With experience in providing a broad range of clinical and consultative services across a range of service settings, you will be skilled in delivering one-on-one, group based and health promotion activities. This will be evident in your ability to support clients, carers and families across the Patient Journey, providing effective assessment and triage, timely referrals, accurate information, coordinated care and prompt follow-up.

Appointment will be subject to a satisfactory Criminal History Check.
